Insurances Accepted by Sullivan Dermatology
Please have your insurances card available when scheduling and checking in for an appointment. If a referral is required, it is mandatory that you obtain one from your primary care physician and bring it with you on the day of your appointment. Otherwise, we may need to reschedule your appointment if one is not provided.
We participate with most insurance plans and as a courtesy, we will verify your benefits and submit claims to your insurance carrier. We cannot guarantee that the verification is accurate so you should always verify with your insurance. Not all services or procedures may be covered by your insurance policy. Our office will do our best to give you an estimate of what charges may be your responsibility at the time of service. If you have an office co-payment, deductible or co-insurance, please be prepared to pay it at the time of your appointment.
Note: Coinsurances and deductibles cannot be waived.
Please be sure to reschedule or cancel any appointments that you are not able attend. If you have more than one “Not Accepted show” where you fail to cancel a visit or multiple re-schedules we may ask you to leave a $50 non-refundable deposit to schedule a your next visit.
