Privacy Policy
1. About This Policy
Tory Sullivan MD PA, doing business as Sullivan Dermatology (“Sullivan Dermatology,” “we,” “us,” or “our”), respects the privacy of our patients, prospective patients, and website visitors. This Policy explains how we collect, use, disclose, and protect personal information through our website, office interactions, forms, telephone calls, email, text messaging, and other services that reference this Policy.

Medical information and HIPAA. Protected health information is also governed by our separate Notice of Privacy Practices and applicable healthcare privacy laws. This Policy does not replace that Notice or authorize uses or disclosures that require a separate authorization. You may request our Notice of Privacy Practices from either office.
2. Information We Collect
Depending on your interactions with us, we may collect the following information:
Contact and identifying information: Your name, mailing address, email address, telephone number, date of birth, signature, and information needed to verify your identity or an authorized representative’s authority.
Appointment and health information: Appointment requests, reasons for seeking care, medical history, medications, allergies, treatment information, photographs, and other information you provide in connection with medical, surgical, cosmetic, or other services.
Billing and insurance information: Insurance coverage, billing details, payment information, and transaction records, including information processed through payment service providers.
Communications and consent records: Questions, messages, correspondence, communication preferences, and records of email or SMS consent and withdrawal, including the date, method, and scope of your consent.
Website and device information: IP address, browser and device type, operating system, device or cookie identifiers, referring pages, pages visited, dates and times of visits, interactions with website features, and approximate location inferred from your IP address.
3. How We Collect Information
We collect information directly when you complete an online contact, scheduling, registration, or consent form; submit a paper form; visit our offices; use a patient portal or telehealth service; or communicate with us by telephone, email, SMS, website messaging, or social media.
We may collect website information automatically through cookies, pixels, tags, local storage, server logs, and similar technologies. These tools are described further below.
We may also receive information from authorized representatives, referring healthcare providers, laboratories, pharmacies, insurers, and vendors supporting services you request, as permitted by law. We do not use purchased or rented contact lists to enroll people in our SMS program.
4. How We Use Information
We use information to respond to inquiries; schedule and confirm appointments; provide and coordinate care; manage patient accounts, billing, payments, and insurance matters; send service-related communications; and support follow-up and office operations.
We may also use information to improve our website and services, evaluate patient experience, maintain consent and opt-out records, protect against fraud or unauthorized activity, and satisfy legal, regulatory, and professional obligations.
Where permitted and with any required consent, we may communicate about Sullivan Dermatology services, educational information, events, or special offers. Consent to patient-service text messages does not automatically authorize promotional text messages.
Research inquiries or participation may be subject to separate consent and privacy documents. This Policy does not enroll you in a study or authorize research disclosures that require separate permission.
5. How We Share Information
We disclose information for the purposes described below, subject to applicable law and the SMS protections in this section. We do not sell or rent patient lists or SMS subscriber lists.
Service providers: We may share information with providers supporting website hosting, scheduling, patient records, billing, payment processing, communications, and technical support. This may include RingCentral and other vendors that operate messaging services on our behalf. Access is limited to the services provided and subject to appropriate confidentiality and security obligations, including business associate agreements when required.
Healthcare and payment coordination: We may share relevant information with healthcare providers, laboratories, pharmacies, insurers, and other parties involved in treatment, payment, or healthcare operations, as permitted by law and explained in our Notice of Privacy Practices.
Website analytics and outreach: Limited device and website-usage information may be disclosed to analytics or advertising vendors to evaluate website performance and measure outreach, subject to applicable law and your choices. This does not authorize disclosure of protected health information for advertising without any required HIPAA authorization. SMS information is excluded from marketing data-sharing as described below.
Legal requirements and business transitions: We may disclose information when legally required or permitted to respond to legal process, protect rights and safety, or complete a lawful practice transfer, merger, or similar transaction. Applicable privacy obligations and the SMS restrictions below continue to apply.
Your direction and research: We may disclose information at your direction or with an appropriate authorization. Research-related disclosures to research teams or study organizations occur only as permitted by applicable privacy requirements and any required authorization or consent.
SMS Data-Sharing Protection
Mobile Opt in, SMS Consent, and phone numbers collected for SMS communication purposes will not be shared with any third party and affiliates for marketing purposes.
We do not sell or rent SMS consent records or mobile numbers collected for our SMS program. Messaging providers and telecommunications carriers may process this information only as needed to operate and support our messaging program, deliver messages, maintain consent and opt-out records, or meet legal obligations. They are not permitted to use it for their own marketing, advertising, or lead generation.
These SMS protections apply throughout this Policy, including to service-provider arrangements and business transfers. Your SMS consent does not authorize another business or affiliate to market to you.
6. Cookies and Similar Technologies
Our website may use cookies and similar technologies to operate website features, remember preferences, understand visitor activity, and evaluate communications or advertising performance. These technologies may be provided by us or by website, analytics, or advertising vendors.
You can manage or delete cookies through your browser settings and use any privacy controls made available on our website. Disabling certain technologies may affect website functionality. We honor privacy choices and preference signals where required by applicable law.
We do not use SMS consent records or phone numbers collected for SMS communications to create advertising audiences or share them for marketing purposes. Accepting cookies or this Policy is not a HIPAA authorization to disclose protected health information.
7. Retention and Security
We retain information for as long as reasonably necessary for the purposes described in this Policy, including providing services, maintaining required medical and business records, documenting consent, honoring opt-outs, resolving disputes, and meeting legal obligations. An opt-out or deletion request does not require destruction of records we must retain by law.
We use reasonable administrative, technical, and physical safeguards designed to protect information against unauthorized access, use, or disclosure. No website, electronic communication, or storage system can be guaranteed completely secure.
8. Your Choices and Privacy Requests
You may contact us to update your contact information, change communication preferences, or ask questions about our privacy practices. Depending on applicable law, you may have rights to request access, correction, deletion, or other limits on the processing or disclosure of personal information. We may verify your identity before responding. Medical-record requests are handled under applicable healthcare privacy and record-retention requirements.
You may unsubscribe from promotional email using the unsubscribe link provided. You may stop text messages as explained in the SMS Terms below. Declining optional marketing or SMS communications does not prevent you from receiving care or purchasing services. We can discuss other available communication methods with you.
9. Children and Authorized Representatives
Our general website is not directed to children under 13. Our practice does provide care to minors and may collect their information through appropriate patient-care processes, consistent with applicable law. We respect the privacy rights of minor patients and involve parents, guardians, or other representatives as legally appropriate.
Please contact us if you believe a child submitted information through our general website inappropriately. We will review and address the situation consistent with applicable privacy and record-retention obligations.
10. External Services and International Visitors
Our website may link to external websites, patient-service platforms, or social media services. Their own privacy notices may apply to their independent activities. Review those notices before submitting information.
Our practice is based in the United States. Information submitted from another country may be processed or stored in the United States, where privacy laws may differ from those in your location.
11. Changes to This Policy
We may update this Policy to reflect changes in our practices or applicable requirements. The revised version will display an updated effective date. We will provide additional notice or obtain new consent when required. A Policy update does not, by itself, expand the types of SMS messages you have agreed to receive.

SMS Terms and Conditions
1. Program and Consent
The Sullivan Dermatology SMS program is operated by Tory Sullivan MD PA d/b/a Sullivan Dermatology. By affirmatively opting in, you agree to receive the types of text messages described in your consent and these Terms at the mobile number you provide. Messages may be sent manually or using automated technology.
Phone numbers collected through SMS consent, mobile opt-in information, and SMS consent records will not be shared with third parties or affiliates for marketing purposes. Operational processing by messaging providers is limited as described in the Privacy Policy above.
2. Types of Messages
Patient-service messages may include responses to your inquiries; appointment confirmations, reminders, and scheduling changes; consultation and registration information; follow-up communications; office updates affecting your appointments; and links to secure patient communications or forms.
Promotional messages, only with separate express written consent, may include information about Sullivan Dermatology cosmetic services, practice events, and special offers. Permission to receive appointment reminders or other patient-service messages does not enroll you in promotional messaging.
We send messages only within the scope of the consent you provide.
3. How to Opt In
You may opt in electronically by selecting an optional, unchecked SMS consent box on a Sullivan Dermatology form, or in writing by completing an SMS consent section on a paper form.
You may also give staff express verbal permission for a specific non-promotional conversation, where permitted. We document that permission and obtain written or electronic consent before enrolling you in recurring informational or promotional messaging. When you text us first, we may respond to your request; that conversation does not automatically enroll you in recurring or promotional messages.
Simply providing your phone number, booking an appointment, submitting a general inquiry, or reading this Policy does not by itself constitute consent to recurring SMS messages. Promotional messaging requires a separate affirmative written or electronic opt-in.
Consent to SMS messages is not a condition of treatment, purchase, or payment. You must be the subscriber or an authorized user of the mobile number provided. Please notify us if your number changes or is reassigned.
4. Message Frequency
Message frequency may vary. Our estimated maximum is up to 6 routine messages per week, including any separately authorized promotional messages. The number depends on your appointments, inquiries, and communication preferences. Additional replies may occur during an active conversation you initiate or request.
5. Message and Data Rates
Message and data rates may apply, depending on your mobile carrier and service plan, including charges for messages sent or received. Contact your carrier for information about your plan. Message delivery depends on your carrier, device, and network availability and is not guaranteed. Carriers are not liable for delayed or undelivered messages.
6. How to Opt Out
Reply STOP to any text message from Sullivan Dermatology to unsubscribe at any time. You may receive one final, non-promotional message confirming your opt-out. After that, we will stop SMS messages to that number unless you affirmatively opt in again.
We also honor other clear requests to stop receiving texts. You may contact either office for assistance. Opting out does not cancel your appointments or change your eligibility for care. Necessary communications can be handled through other appropriate channels.
7. Help and Support

8. Privacy and Medical Communications
Standard SMS is not a secure patient portal and may be accessible to others with access to your phone or messaging account. Avoid sending sensitive medical information, photographs, payment card information, or identification documents through ordinary text messages. Contact our office for an appropriate secure communication method.
SMS consent is not a waiver of your healthcare privacy rights or a substitute for any required medical authorization. Messages are not continuously monitored and should not be used for urgent medical concerns. For a medical emergency, call 911.
9. Standard Messaging Disclosures and Policy Links
Message and data rates may apply. Reply STOP to opt out. Reply HELP for assistance. Message frequency may vary; estimated maximum of up to 6 routine messages per week, with additional replies during an active conversation you initiate or request.
